A conscious choking victim should be positioned sitting upright to effectively deliver back blows. This position allows gravity to assist in dislodging the obstruction from the airway when delivering back blows between the shoulder blades. When a person sits upright, their airway is more easily accessible, and the lungs are positioned in a way that enhances the effectiveness of the back blows.

Positioning a conscious choking victim in any other manner, such as on their side, flat on their back, or in a reclined position, may hinder the ability to provide effective back blows. For example, lying flat might not facilitate the same gravity effect and may obstruct airflow rather than alleviate the choking. Thus, the sitting upright position is the preferred and most effective choice.
